Sports Physicals Available at Dutton/Brady School on August 7th

Physical and concussion protocol must be met for all athletic activities before a student can participate in ANY practice or game.

Logan Health will be at Dutton/Brady School on Wednesday, August 7th from 8:00 am to 1:00 pm to perform physicals.  If a student is interested in any sport for the year, this is a great time to take care of this requirement to play.  We only offer this option one time a year.  The physicals are only $25.00 per person (but are not insurance applicable).  If the student is coming in to get their physical and a parent is not going to be present, a consent form must be filled out and signed prior to attending,  Logan Health Consent Form.  The MHSA physical form must be used for these physicals and must be signed by both the student and the parent prior to the physical.

EACH YEAR all student athletes and parents must sign and initial the MHSA Student-Athlete & Parent/Legal Guardian Concussion Statement before any practice or participation in any sport.
